Enrollment Forms Checklist
ALL forms must be completely filled out and submitted to complete enrollment.
Your child will not be considered enrolled in the class until these forms have been submitted.
The non-refundable registration fee and May tuition payment are due at the time of registration.
They can be mailed to MUM Preschool, 8600 Mentor Ave., Mentor, OH 44060 or dropped off to the church office.

Child Enrollment and Health Information
It MUST be turned in 30 days from the first day of school. It MUST be signed by the physician and a copy of your child’s shot records attached.
